Universal Gymnastics Booster Club Team
Agreement
This form holds as an agreement between Universal
Gymnastics Booster Club and
Mr. or Mrs.
_________________________________________________________
The parent(s) of
_________________________________________________________,
a gymnast at Universal Gymnastics.
The parent or legal guardian agrees to the
following:
- Attend the Booster Club’s general membership
meetings.
- Pay to Universal Gymnastics Booster Club dues
of $50.00 per calendar year due by December 1st.
- If your account has a negative or in red
balance, you will be notified in writing of your account status and
required to pay the balance and any late fees within two weeks of
such notice. Failure to pay may result in the gymnast(s) being
prohibited from competing with the team.
- Follow the volunteering guidelines specified
in the Bylaws in order to be counted towards the fundraised monies.
- Give written notice on a timely manner upon
termination of the team and the Booster Club. All unused “personal”
monies will be refunded and “fundraised” monies will be dispersed to
the general fund.
- Keep the Booster Club informed of any personal
changes as they occur. For example; address, phone numbers, email
contact, and emergency contact.

Universal Gymnastics Booster Club Team Agreement.
